Names and gender of translations

As I am reading I can’t understand why the character names keep changing. Is it a nickname or title or last name initial then first name?
The use of he or she seems to be wrong a lot of the time. In most western languages there is strict use of masculine and feminine references but not in the translated novels from chinese to english.
